In our Physical Medicine and Rehabilitation Clinic, otherwise known as physiatry services, UAMS experts provide treatment of the whole patient so that you can reach your full potential and live life to the fullest.

Appointments in the Physical Medicine and Rehabilitation Clinic are by referral only. Please contact your primary care doctor or visit one of our doctors in the Center for Primary Care to obtain a referral.

What should I bring to the appointment?
For your first appointment, please bring a picture ID and your insurance card. The UAMS medical staff will also need information about any medications you're taking, including over the counter medications. You should also bring any medical records you can provide. Also, bring a completed copy of the Patient Health Questionnaire sent to you after you set your first appointment.
For future appointments, please advise us of any changes in your insurance information or your medications. This clinic also requires that all new patients pay for their first visit at the appointment.
How do I request a prescription refill or contact a nurse from the clinic?
Call the clinic at 501-221-1311 to request a prescription refill, or you can request prescription refills during your appointment. Your pharmacy can fax a prescription refill request to the Clinic at 225-0627. You may need to provide your pharmacy's name and contact information during the phone call.
If you originally filled your prescription at the UAMS Outpatient Pharmacy, you can also refill your prescription online. If you need to contact a nurse in the clinic, call the clinic at 501-221-1311. You may need to provide your doctor's name to speak with the correct nurse.
