Have you ever had problems getting a clear diagnosis or an appointment in the clinic you need for your condition? If so, UAMS offers a Diagnostic Clinic to ensure that you receive efficient and effective medical treatment.
We recognize the need you have to see a doctor in a timely manner and get answers to your questions. Our Diagnostic Clinic will help you get the medical care that you need to treat whatever condition you may have. Your doctor may refer you to our clinic, or please feel free to contact us directly if you don't have a referral.
Charles Smith, M.D., professor and executive associate dean for clinical affairs in the College of Medicine, leads our Diagnostic Clinic at UAMS. Dr. Smith is board certified in family and preventive medicine and has more than 30 years of clinical experience.
At your appointment, we will conduct a thorough investigation of your condition and order the necessary tests to make a correct diagnosis. Once a diagnosis has been reached, our doctor will either refer you to the appropriate subspecialist at UAMS or recommend an initial approach to treatment.
You will probably return to the clinic for at least one follow-up visit. However, our Diagnostic Clinic does not function as a primary care facility or a place where you receive ongoing medical care. Our goal is to make sure that you have received the correct diagnosis and that you are receiving the best medical care possible for your condition. If your situation is complex, our caring and compassionate doctors will arrange for you to have further consultations with UAMS specialists to ensure that you are moving toward relief or resolution of your medical issues.
The Diagnostic Clinic is a self-referral clinic. Although UAMS does not require a referral for this clinic, some insurance providers require a referral for specialty care. Please check with your insurance provider before requesting an appointment.
The quickest way to request an appointment is through our online appointment service.
If you prefer to call and request an appointment, please call 501-686-6560 for new patients, returning patients and referral appointments.
What should I bring to the appointment?
For your first appointment, please arrive 30 minutes early to help make sure all paperwork is completed. Please bring a picture ID and your insurance card. The UAMS medical staff will also need information about any medications you're taking, including over-the-counter medications. You should also bring any medical records you can provide, including test results from other clinics.
For future appointments, please advise us of any changes in your insurance information or your medications.
How do I request a prescription refill or contact a nurse from the clinic?
Call the Family Medical Center at 501-686-6560, and follow the menu prompts to request a refill. You should provide the name and telephone number for the pharmacy that will refill the prescription. If you originally filled your prescription at the UAMS Outpatient Pharmacy, you can also refill your prescription online.
To speak with a nurse, call the center at 501-686-6560 and provide the name of your doctor.
Dr. Charlie Smith, a primary care physician with the UAMS Family Medical Center, enjoys building relationships with his patients to optimize their health.
