Read the UAMS Confidentiality Policy and complete the MD-LINK Access Request Form  and fax to 501-686-6013.  You will receive an email confirmation that your request has been received.  You will receive another email to confirm access and a confirmation code to activate your account.

While a written authorization may not be necessary for treatment/payment purposes, UAMS is requiring your patients' written authorization granting you on-line access to their medical records.  On-line access will be available on patients who present to UAMS after January 2006.   If you would like on-line access to records on patients seen before this date or on patients who have not yet authorized on-line access, have your patient complete the Patient Authorization Form and fax (501-686-6013) or mail to UAMS Physicians Relations, 4301 W. Markham, #727, Little Rock, AR  72205. 

A patient's information will be available on-line for MD-Link access for approximately twelve months from the patient's appointment/admission.  Please call Medical Records at 501-603-1520 for records not available on-line.
